Survey concerning Aging Aircraft for my research project

Ladies and Gentlemen,

Attached (hopefully) is a word document that is a survey concerning Aging Aircraft and is a data gathering device I'm attempting to use to finish up my Graduate Research Project for a Masters of Aeronautical Science from Embry-Riddle. If you have the time, please look it over and fill it out and send it back to me off-line using my personal email address below:

Feel free to make comments as you like and all submittals will be kept confidential with no personal information being used by either myself or anyone else. To all

Juds form as of 9:50 Tuesday EST will not work as he had anticipated as a form. I've informed him how to correct this and hopefully you all will try again to fill out the form when he notifies us he has it corrected.

If you have downloaded the form already and want to fix the form you have so that it works properly do the following:

Open the form. Under the Tools menu select Protect Document. Click the radio button next to Protect form. It is not necessary to enter an password and you should leve that blank for Jud, and close the box. That is it. Your form will work as you wish.

BTW if you want to leave Jud your name and address for the results the first page of the survey indicates you can you must first enter your name and address, then protect the form. If you unprotect the form after taking the survey in order to enter you name and address, do not reprotect the form or all you choices will be erased.

I think I have captured Bruce's information and attached the correct form that you should be able to fill out the requested data. Thanks for the help Bruce! If anyone has any questions, feel free to contact me.
