Some good reading from Mike Busch at AvWeb. My first engine overhaul was due to a mis-adjusted actuator arm when the pull-type starter was replaced.

Aeroperu 603 (and many other post-mx failures) was assisted by improper/incomplete pre-flight inspection..... not entirely caused by improper mx. But many of the failures I've experienced in airplanes have been subsequent to faulty mx. (Thinking now of the oil leak problems I wrote about at the Las Vegas convention.)
